Chief Executive Donald Tsang announced on Friday that Lam replaces Henry Tang who resigned two days ago to mull running for next year's Chief Executive election.
Raymond Tam moves from the post of director of the Chief Executive's Office to take over from Lam as the secretary for constitutional and mainland affairs.
Gabriel Leung, former undersecretary for food and health, moves to take over as director in the chief executive's office.
Tsang said each of the appointees was the best candidate for the job.
He said the trio share the qualities of great integrity, dependability, are capable and passionate about serving Hong Kong.
Tsang announced the reshuffle on Friday at a press conference after the Central Government approved Henry Tang's resignation and the appointment of the two principal officials on Tsang's recommendation.
